Duties Highlights for a Licensed Practical Nurse (LPN):

  • Provide skilled nursing care to patients
  • Provide meaningful education to patients
  • Support the RN Case Manager
  • Execute tailored Care Plans

Qualifications for Homecare Registered Nurse (RN) Case Manager:

  • Proof of Flu vaccination
  • Current NH LPN license
  • Home Care experience beneficial, although we will train the right individual
  • Automobile insurance
  • Physical within the last year
  • Two-step TB test
