Position Summary
Aligned Hospice is seeking an experienced Hospice Medical Social Worker (MSW) to join our interdisciplinary team on a per diem basis.
This position is ideal for a social worker with a true hospice heart. We are seeking someone who understands the privilege of serving patients and families during the end-of-life journey and who is passionate about helping patients achieve a "good death" consistent with their goals, values, and wishes.
The successful candidate will be a strong communicator, highly organized, emotionally intelligent, and able to work both independently and collaboratively as part of an interdisciplinary team. We are seeking a clinician who is trustworthy, dependable, professional, and comfortable managing a caseload autonomously while maintaining frequent communication with patients, families, facilities, and team members.
Essential Responsibilities
- Complete psychosocial assessments and ongoing reassessments for hospice patients and families.
- Provide supportive counseling, anticipatory grief support, and emotional support throughout the hospice journey.
- Educate patients and families regarding disease progression, end-of-life issues, community resources, and hospice services.
- Facilitate goals-of-care discussions and assist families in navigating complex decisions.
- Collaborate closely with physicians, nurses, chaplains, aides, and facility staff to ensure coordinated patient care.
- Participate actively in interdisciplinary team meetings and care planning.
- Advocate for patients and families while promoting patient-centered care.
- Maintain timely and accurate clinical documentation.
- Develop and maintain relationships with community resources and care partners.
- Provide bereavement support and referrals as appropriate.
- Other duties as assigned.
Qualifications
Required
- Master's Degree in Social Work (MSW) from a CSWE-accredited program.
- Minimum one (1) year of hospice social work experience.
- Demonstrated understanding of hospice philosophy, grief, loss, end-of-life care, and family systems.
- Excellent verbal and written communication skills.
- Strong assessment, counseling, and documentation skills.
- Ability to work independently, prioritize responsibilities, and manage time effectively.
- Ability to build rapport with patients, families, facilities, and referral partners.
- Valid driver's license, reliable transportation, and current automobile insurance.
