Requirements

Roles & Responsibilities

• Perform comprehensive patient assessments in home health and hospice settings
• Develop, implement, and evaluate individualized plans of care
• Provide skilled nursing services and patient education
• Monitor patient progress and communicate changes to physicians and care teams
• Coordinate care with interdisciplinary healthcare professionals
• Manage medication administration and treatment plans
• Educate patients and caregivers regarding disease management and care goals
• Maintain accurate and timely electronic medical record documentation
• Support end-of-life care and hospice patients with compassion and professionalism
• Promote patient safety, independence, and quality of life in the home setting

Qualifications

• Active New Hampshire Registered Nurse (RN) License required
• Current American Heart Association (AHA) BLS Certification required
• Minimum 18 months of current Home Health experience required
• Previous Hospice experience preferred
• 2–5 years of Medical-Surgical nursing experience preferred
• Strong assessment, clinical judgment, and care coordination skills
• Ability to work independently with minimal supervision
• Computer proficiency and EMR documentation experience required
• Valid Driver’s License required
• Current Auto Insurance required
• Reliable transportation required
