Occupational Therapist (OT) – PRNAscend Home Health | Southern Idaho

Ascend Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our team on a PRN basis serving patients throughout Southern Idaho.

This role offers flexible scheduling and the opportunity to make a meaningful impact by helping patients regain independence, improve daily functioning, and safely remain in the comfort of their homes.

Responsibilities:

• Evaluate and treat patients in their homes according to physician orders and plan of care
• Develop individualized therapy treatment plans focused on improving daily living skills and independence
• Educate patients, families, and caregivers on therapy techniques, safety, and adaptive equipment
• Collaborate with nurses, therapists, physicians, and the interdisciplinary care team
• Complete timely and accurate documentation

Qualifications:

• Current Occupational Therapist license required
• Home health experience preferred, but not required
• Strong communication and organizational skills
• Compassionate, dependable, and patient-focused
• Valid driver's license, reliable transportation, and current auto insurance required

Position Details:

• PRN / Flexible scheduling
• Coverage area throughout Southern Idaho
• Competitive pay per visit DOE (depending on experience)
