Responsibilities
The LPN Medication Reconciliation Nurse will perform medication reconciliation of orders with our contracted facilities’ Medication Administration Records (MARs) and Electronic Medical Records (EMR) to ensure accuracy and avoid medication errors. They will coordinate the process for any corrections and ensure follow-up was completed.
- Complete monthly medication reconciliation of facility MARs with InnovAge medication list.
- Communicates medication-related issues to all necessary parties.
- Pulls necessary reports throughout the month to ensure accurate orders.
- Documents in EMR and other applications as needed.
- Visit facilities monthly to obtain MARs, communicate with staff and deliver reconciled orders.
- Knowledge of medical terminology and pharmaceutical abbreviations.
- Knowledge of pharmaceutical generic and trade names and their dosage forms, and the ability to cross-reference common drugs from memory.
- Requires intense and consistent concentration with attention to detail, speed and accuracy.
- Ability to work independently and follow-through on assignments with minimal supervision.
- Telephone and verbal skills required to communicate clearly and effectively with pharmacists, nurses, physicians.
REQUIRED
- Current LPN license to practice in the state
- 3+ yrs clinical experience in a health care/long-term care setting
- Demonstrates clinical assessment skills at the level necessary to meet the job requirements
- Knowledgeable of nursing and medical practices and procedures, laws, regulations and guidelines that pertain to PACE
- Effective interpersonal skills and the ability to work with an interdisciplinary care team
- Ability to plan, organize, develop, implement and interpret the programs, goals, objectives, and policies and procedures that are necessary for providing quality care
- Excellent computer technology including e-mail use
- Create a positive and effective relationship with associates, Participants, family members and external agencies and organizations
