The schedule for this role is Monday - Friday 8 am to 5 pm.
This is a full-time professional position. The LPN provides routine and prn visits for assigned patients under the supervision of the assigned RNCM and or IDG Team Leader. The LPN may also provide continuous nursing care for patients during a short-term period of crisis on a rotating basis and provide phone support to patients and their caregivers for their assigned team(s).
Responsibilities what you do
- Provide and document safe, competent, compassionate hospice nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by the physician orders and the hospice plan of care. Patient response to care is reported to a registered nurse for evaluation, intervention and modification of the plan of care.
- Provide and document telephone support to patients, families, physicians and partners to assist in immediate needs that may be able to be met at a distance. These needs may include, but are not limited to: Physician communication, obtaining orders, follow up on changes in patient condition and other tasks to assist in patient care from the office setting within the LPN scope of practice. LPNs will seek guidance and or direction for any situation that requires RN evaluation and modification of the established POC..
- Responsible for reviewing supplemental orders, chart audits and other office duties as assigned.
- Participate in own professional development by maintaining required nursing competencies and attending educational offerings.
- Responsible for providing and documenting appropriate and relevant end of life information and education to patients and their families in compliance with current Centers for Medicare and Medicaid Services CMS Conditions of Participation (CoPs).
- Responsible for performing facility chart audits, as assigned.
- Responsible for professionally managing time, people and technology.
- Monitors and manages the appropriate use of patients’ medical equipment, supplies and medications.
- Collaborates with other Interdisciplinary Group (IDG) members to determine the patient’s level of care and coordinate the appropriate utilization of contract services.
- Participate in the identification of clinical or operational performance improvement opportunities and assists in performance improvement activities.
- Actively participates in the orientation and training of new employees of all disciplines.
- Works collaboratively with IDG and other Transitions LifeCare (TL) departments in achieving patient care goals.
- Performs other duties as assigned by supervisor.

Requirements what you need
- Licensed Practical Nurse with current NC license.
- Minimum 2 years recent clinical experience. Hospice and/or home health or nursing home experience or equivalent preferred.
- Hospice Nurse Certification preferred.
- Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards.
- Competent computer skills.
- Must have reliable transportation.
- Current CPR certification.
- Must be able to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide care to all patients.
- May be required to lift and/or carry items up to 50 pounds.
- Must have sensory abilities to complete physical assessment, communicate with patient/family care providers and physician and to complete the reporting and follow-up documentation responsibilities of the job. Workdays and scheduled hours with weekend and holiday commitments as outlined in Employee Information Agreement.
