This position focuses on providing advanced social services work to children and adults with serious and persistent mental illness. As a Blended Case Manager, you will perform the following duties:
- Mandated Reporting: Serve as a mandated reporter at all times
- Case Management: Provide intensive case management services to individuals with serious and persistent mental illness
- Service Monitoring: Maintain required billable service units and enter monthly data by the established deadline
- Documentation: Complete all necessary daily documentation such as forms, case notes, reports, and correspondence
- Information Gathering: Obtain information from consumers, families, and others to complete mental health assessments
- Service Planning: Develop and implement individual service plans based on assessments and interviews
Work Schedule and Additional Information:
- Full-time employment, 35 hours per week
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch. Occasional overtime and travel may be required.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- Two years of experience as a County Caseworker 2; or
- A bachelor's degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and three years of professional social casework experience in a public or private social services agency; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
Other Requirements:
- PA residency requirement is currently waived for this title.
- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
