The Patient Care Coordinator position with the Department of Human Services offers a chance to make a strong impact on patient well-being at Warren State Hospital. This position supports the review and evaluation of patient records to ensure that admissions and continued stays meet medical and regulatory standards. The role requires careful study of rules, coordination with hospital teams, and consistent review of patient information. As a Patient Care Coordinator, you will perform the following duties:
- Utilization Review: Review patient records to determine the medical need for admission and continued stay
- Length Monitoring: Track and evaluate patient length of stay and ensure timely review of extended stays
- Record Evaluation: Analyze documentation to support compliance with state and federal requirements
- Team Coordination: Work with hospital staff to support communication, treatment planning, and documentation needs
- Data Reporting: Prepare summaries and reports for committees, leadership, and program monitoring activities

Work Schedule and Additional Information:
- Full-time employment, 37.5 hours per week
- Work hours are 1st shift (8:00 AM to 4:00 PM), Monday - Friday, with a 30-minute lunch.
- Overtime and travel: As needed.
- Telework: You will not have the option to telework in this position.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- One year as a Psychiatric Nurse 2 in a state hospital or a Nurse 2 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) in a State General Hospital or Restoration Center; or
- Three years of professional psychiatric nursing or general nursing, including one year at an advanced level in a state hospital, general hospital or Restoration Center; and graduation from an approved school of professional nursing; or
- Two years of professional social or rehabilitative work, including one year of clinical experience in a supervisory, administrative or consultative capacity in a state hospital, state general hospital or Restoration Center; and a bachelor's degree with major course work in social work, nursing, health care administration, behavioral sciences or related fields; or
- Any equivalent combination of experience and training.
