Position Type: PRN (As Needed)
Schedule: Evening and Weekend Coverage

Position Overview

Ohio’s Hospice is seeking a compassionate and dedicated LPN or RN to provide after-hours nursing support for patients receiving hospice care in the New Philadelphia and Wooster communities. This driving position offers an opportunity to make a meaningful difference in the lives of patients and families by delivering exceptional care during some of their most important moments.

The ideal candidate is committed to hospice philosophy, values patient-centered care, and is comfortable working independently while collaborating with an interdisciplinary care team.

Schedule Expectations
  • Evening shifts: 4:00 PM – 12:00 AM, one to three shifts per week
  • Weekend coverage: 8:00 PM – 8:00 AM every other weekend
  • Participation in a rotating holiday schedule is required
  • Travel throughout the assigned service area is expected
Key Responsibilities
  • Deliver skilled nursing care to hospice patients in homes, assisted living communities, and other care settings
  • Assess patient needs and provide interventions that support comfort, dignity, and quality of life
  • Collaborate with Registered Nurses and other members of the hospice care team to implement individualized care plans
  • Educate and support patients, caregivers, and family members regarding care needs and symptom management
  • Maintain timely and accurate electronic documentation
  • Respond professionally and compassionately to changing patient conditions
Qualifications
  • Current Ohio LPN or RN license in good standing
  • Minimum of one year of nursing experience preferred
  • Current CPR certification
  • Proficiency with electronic documentation and basic computer applications
  • Ability to travel independently and safely during daytime, evening, overnight, and adverse weather conditions
  • Valid driver's license with an acceptable driving record
  • Proof of state-required automobile insurance coverage
  • Successful completion of a criminal background check
  • Successful completion of a 10-panel drug screening
