We are seeking a compassionate and skilled Registered Nurse (RN) Care Manager to provide high-quality hospice care to patients and families throughout Licking County. This full-time position offers the opportunity to make a meaningful difference by supporting patients during their end-of-life journey as part of an interdisciplinary care team.


Position Details

  • Full-time, 40 hours per week
  • Schedule: Monday–Friday, 8:00 a.m. – 4:30 p.m.
  • Rotating holidays required, as well as every 8th weekend rotation.
  • Driving position serving Licking County
  • Collaboration with interdisciplinary team members is essential

Primary Responsibilities

The RN Care Manager is responsible for planning, coordinating, and delivering patient care using the nursing process, including assessment, planning, implementation, intervention, and evaluation. The RN Care Manager works closely with patients, families, physicians, and other healthcare professionals to ensure compassionate, patient-centered care while maintaining professional nursing standards.

Qualifications

  • Graduate of an accredited nursing program
  • Current Registered Nurse (RN) license in the State of Ohio in good standing
  • Two years of acute care nursing experience preferred
  • Hospice and Palliative Nursing Certification (CHPN) preferred
  • Certification preparation and support are available for eligible employees
  • Current CPR certification
  • Basic computer skills and ability to accurately document patient care
  • Ability to travel and drive in various weather and road conditions
  • Valid driver's license and safe driving record
  • State minimum automobile insurance coverage
  • Ability to pass a criminal background check
  • Ability to pass a 10-panel drug screening
