Licensed Vocational Nurse

Key Duties and Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participates in coordination of palliative care services, appropriately reporting the identified needs to the interdisciplinary group.
• Uses equipment and supplies effectively and efficiently.
• Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
• Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
• Performs other duties as assigned by the RN or Lead NP

Education and Experience

• Currently licensed as Vocational Nurse in the state of California.
• One (1) to three (3) years of nursing experience. Community health/hospice or medical/surgical and palliative care experience is preferred.
• Possesses and maintains current CPR certification as required by organizational policy.

• Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to life supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
• Thrives in an unstructured, start-up environment.
• Self-starter that can work independently and collaboratively, prioritize tasks and has initiative and excitement to take on unfamiliar tasks.
• Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks
• Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities
• Working knowledge of company policies, procedures, and operations
• Excellent composition, grammar, and business language skills
• Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members.

• Excellent judgment in handling confidential and sensitive information

• Understand the philosophy of palliative. Also understands needs of the terminally ill.

Licenses/Certifications

• Currently Licensed Nurse in the state of California
• Current CPR certification as required by the organizational policy
