Key Duties and Responsibilities
- Provides direct patient care as defined in State Nurse Practice Act.
- Implements current nursing practice following a comprehensive assessment and the plan of care.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Participates in coordination of palliative care services, appropriately reporting the identified needs to the interdisciplinary group.
- Uses equipment and supplies effectively and efficiently.
- Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
- Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
- Performs other duties as assigned by the RN or Lead NP
Education and Experience
- Currently licensed as Vocational Nurse in the state of California.
- One (1) to three (3) years of nursing experience. Community health/hospice or medical/surgical and palliative care experience is preferred.
- Possesses and maintains current CPR certification as required by organizational policy.

- Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to life supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
- Thrives in an unstructured, start-up environment.
- Self-starter that can work independently and collaboratively, prioritize tasks and has initiative and excitement to take on unfamiliar tasks.
- Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks
- Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities
- Working knowledge of company policies, procedures, and operations
- Excellent composition, grammar, and business language skills
- Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members.
- Creative, flexible, well organized, resourceful, and detail-oriented
- Excellent judgment in handling confidential and sensitive information
- Ability to work independently, set priorities and handle multiple tasks with a high level of efficiency
- Establishing and maintaining cooperative working relationships with others
- Ability to work across locations and time zones
- Understand the philosophy of palliative. Also understands needs of the terminally ill.
Licenses/Certifications
- Currently Licensed Nurse in the state of California
- Current CPR certification as required by the organizational policy
