Job Purpose:

Our next Permanency Liaison is responsible for overseeing all case management tasks related to children exiting out of home care and achieving legal permanency (reunification, adoption, or guardianship). The Permanency Liaison is assigned to all children in out of home care and work alongside the case management to ensure children and families are supported on their journey to finding permanence.

Duties:

  • Provides technical and supportive assistance to ensure timely response and coordination of documentation and data driven actions connected to timely permanency outcomes in eWiSACWIS.
  • Supports case management team in drafting and completing documents required for initiating permanency legal processes, which includes supporting in completion of RPR/ROG referrals, gathering all required documents for TPR referrals and submit to court, ensuring that all required documents are submitted with TPR referral.
  • Ensures adoptive resource or proposed guardian(s) meets all requirements (adoption/licensing approval completed training, kinship, or SSI).
  • Attends home visits to discuss permanency options/processes and meets with families to complete necessary documents to achieve permanency.
  • Recommends legal permanency options using State standards in accordance with federal, state, and agency policy.
  • Collaborates with child welfare colleagues to best meet the needs of the families we serve.
  • Organizes time effectively, maximizes use of available resources, successfully balances the competing demands of multiple projects by setting priorities and communication and meeting deadlines.
  • Demonstrates adaptability, recovers quickly when progress is stalled by obstacles; responds with flexibility and resilience when faced with multiple demands, shifting priorities and rapid change.
  • Monitors progress of permanency (reunification, TOG, TPR, adoption) and documents ASFA per policy for compliance.
  • Communicates effectively and with high quality through verbal, non-verbal, and written communication with accurate and relevant information in a timely manner.

Qualifications:

  • Bachelor's Degree in Social Work (BSW) from an accredited program or a related field is required.
  • It is preferred that BSWs are certified by the state of Wisconsin as a Social Worker within 18 months of hire.
  • It is preferred that Non BSWs complete approved training within 24 months of hire and State certification by 36 months of hire.
  • Experience in Child Welfare and/or a related human services field preferred.
  • Knowledge of community resources and services available to program participants.
  • Demonstrates knowledge of the child welfare system, systemic barriers and encourages innovation.
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g. Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Demonstrates an understanding of the cultural differences among diverse groups and the need to adapt service provision to match these differences in respectful ways.

  • Public Service Loan Forgiveness (PSLF) - Wellpoint employees may be eligible for loan forgiveness under the Public Service Loan Forgiveness program.
  • Interaction with personnel, clients, family members, visitors, volunteers, interns, and vendors under all conditions and circumstances.
  • Fast paced, flexible work schedule includes evenings and some weekends to accommodate program need.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
