Job Description

We are seeking professional and compassionate Qualified Professionals (QP)/Associate Professional (AP) for our growing Intensive In-home team. The QP/AP will be responsible for the development, implementation, monitoring, and revision of the Person-Centered Plan (PCP) in conjunction with the interdisciplinary healthcare team. This position works with the child/adolescent population. Providing Skill building, and psycho-education to promote behavioral modification.

Responsibilities:

Supporting the consumer in the development of various skill-building activities such as daily and conflict resolution skills, socialization skills, appropriate behavior, and anger management skills.
Performing case management functions of linking and arranging for services and referrals.
Providing care coordination by working closely with other clinical/professional staff to maintain communication and provide feedback.
Creating and implementing PCPs, expeditiously.
Facilitating relationships and serving as a link between the company, parents/guardians, schools, and local agencies.
Team Oriented.

Job Type: Full-time

Pay: $15.00 - $20.00 per hour
