The Certified Nurse Assistant (CNA’s) is a dedicated compassionate person who will provide direct support to individuals across multiple homes with a primary focus on assisting with medical appointments and care. The ideal candidate will be skilled in delivering basic care, completing documentation, and ensuring a safe and clean environment. When CNA responsibilities have been met, the candidate will assume the responsibilities of a Direct Support Professional and be available to cover open hours due to vacation, illness, vacancies, etc. between the hours of 6:30 am to 6:30 pm. This position guarantees 40 hours per week, with 2 days off. Shift hours are not guaranteed Monday to Friday.

1. Be a positive role model for co-workers and employees.

2. Create an atmosphere that is warm, caring, and home-like with minimum friction among individuals and staff.

3. Coordination and transportation of individuals to medical appointments for their assigned team.

4. Support and teach individuals in all aspects of self-care activities as they require.

5. Work cooperatively and collaboratively with individuals supported, their families, and staff.

6. Assist individuals in community activities, i.e. budgeting, menu planning and shopping, accessing recreational activities, etc.

7. Advocate for each individual's rights and encourage them to make good decisions.

8. Promote integration into the neighborhood and the community.

9. Communicate well with the individuals they support, other staff and supervisors, parents, guardians and other agencies.

10. Monitor and/or maintain the cleanliness and safety conditions of the residence and vehicles.

11. Generate and supervise daily activity schedules and assure opportunities exist for a meaningful choice in a timely manner.

12. Work closely with supported individuals, family members, consultants and other related staff in planning and implementation of Individual Plans.

13. Follow established procedures and document information consistent with the expectations outlined in the Individual Plan.

14. Reflect values of the organization and develop person-centered principles.

15. Represent the agency to organizations, individuals and government officials in a positive manner.

16. Staff may be required to perform additional duties not specifically listed or implied here as situations arise or as assigned by a supervisor.

17. Assure confidential information is upheld in accordance with Woodhaven policies, procedures, DMH regulations, Federal and State laws.

18. Woodhaven reserves the right to alter your work schedule and work location to accommodate individuals supported and staff needs.

19. Have reliable transportation and arrive on time for all scheduled shifts, complying with Woodhaven call out procedures

20. Annual renewal of certifications and in-service required courses

21. Maintain a record free of Department of Mental Health disqualifying felonies

22. Unless a request for exception has been approved with the Department of Mental Health, employees will be removed from Payroll effective immediately

Requirements:

Essential Qualifications

  • Education:
    • High school diploma or equivalent required.
  • Certifications:
    • Valid State CNA License
    • Must successfully complete an initial orientation including CPR, Positive Behavior Interventions, and a medication administration course.
    • Valid Missouri chauffeur's license required within the first two weeks of employment; must maintain acceptable driving status according to Procedure No. 1-8-3, Woodhaven Vehicle Use.
  • Other Requirements:
    • Ability to work cooperatively and collaboratively with individuals supported, heir families, and staff.
    • Experience working as a CNA
    • Strong attention to detail and communication
    • Must have no felony convictions listed on the Department of Mental Health disqualification registry unless an approved exception exists.

    Essential Physical Requirements

  • This is primarily physical work requiring exertion of 50 pounds or more of force and frequent fine motor skills.
  • Tasks require grasping, repetitive motions, vocal communication, and hearing.
  • Visual acuity is needed for data preparation, machine operation, and general observation.
  • Some homes may require lifting a minimum of 50 pounds. Please consult with the supervisor for specific requirements.
