Experienced Home Care Intake Specialist (Spanish)

Job Summary:

Crown Home Care is seeking an experienced full-time Intake Specialist for our Brooklyn office. This position is an essential part of our team, working closely with Community Outreach Representatives and potential clients to facilitate the setup of care for members enrolled with Crown Home Care. This role is pivotal in processing referrals from various sources to ensure timely enrollment in our services.

Responsibilities:

• Understand all services offered by Crown Home Care to best serve patients and generate daily referrals and secure their enrollment with the agency.
• Act as a liaison between the agency and partner contracts, coordinating member eligibility and care initiation.
• Assist active patients in gathering necessary documentation and help new patients obtain Medicaid or direct them to third-party services for Medicaid activation.
• Maintain regular follow-ups with potential patients and contracts to ensure smooth processing of intake and enrollment and be able to secure their enrollments.
• Manage the entire referral intake process, from initial contact to admission.
• Analyze weekly reports from agency contacts, completing all required documentation.
• Maintain and manage an active caseload, ensuring timely initiation of services for potential patients.
• Schedule and manage assessments and reassessments for applicants and enrollees through HHA eXchange.
• Ensure timely communication with the Intake Director, team members, and the Marketing team.
• Facilitate constant information exchange across various departments to coordinate assessments and collect necessary patient data.
• Document and track all communications related to authorizations, both past and present.

Job Requirements:

• Bilingual proficiency (spoken & written) in Spanish.
• High school diploma or GED equivalent. Some college preferred.
• Two years of intake experience required.
• Experience with HHA eXchange, insurance portals, and other CRM systems for intake.
