Responsibilities

  • Deliver patient care within scope of practice as an LPN on site.

  • Document patient status accurately in electronic health records.

  • Maintain high standards of infection control and safety compliance.

  • Meet performance targets using established pipelines and metrics.

  • Assist with medication administration under supervision and policy.

  • Coordinate care plans with healthcare team to meet targets and outcomes.

  • Monitor and report patient progress to support rapid decision-making.

  • Collaborate with teammates while owning individual accountability for tasks.

  • Maintain confidentiality and professionalism in all interactions.

  • Implement evidence-based practices to improve patient outcomes.

  • Engage in continuous learning to grow toward advanced nursing responsibilities.

  • Participate in on-site training and development sessions regularly.


Experience/Qualifications

  • Licensed Practical Nurse (LPN) license in applicable state.

  • Deliver direct patient care within LPN scope on-site per policy.
