You will enjoy the autonomy of independent practice while being supported by a collaborative interdisciplinary team. Your patients will typically be adults and seniors with chronic conditions, post-acute needs, or rehabilitation goals who benefit from receiving care in the comfort of their homes.\n\nTypical responsibilities include comprehensive patient assessments, wound and ostomy care, medication reconciliation and administration, IV therapy as indicated, disease management education, and monitoring of vital signs and symptoms. You will coordinate closely with physicians, therapists, social workers, and home health aides to develop and implement individualized care plans, ensuring continuity and quality of care. Patient and caregiver teaching is central to the role, empowering families to safely manage care between visits.\n\nA typical day can include a mix of start-of-care visits, routine follow-ups, and occasional resumption or discharge visits. You will travel between patient homes within a defined local radius, using your clinical judgment to prioritize care needs and adjust plans in real time. Documentation is completed using an electronic medical record system, often via laptop or tablet, allowing you to chart efficiently in the field or at home. Visit volume is managed to support safe, high-quality care and appropriate time for documentation and travel.\n\nShifts are generally scheduled during daytime hours with some flexibility to accommodate patient needs and personal preferences, depending on agency requirements. Weekend or on-call rotations may be part of the schedule, shared among the nursing team. Caseloads are structured to balance clinical complexity, mileage, and visit numbers, so you can focus on meaningful patient interactions instead of feeling rushed.\n\nThis role is an excellent fit for nurses who value building long-term relationships, seeing the impact of their care in a patient’s own environment, and practicing a broad range of clinical skills. Working in a home setting allows you to see the whole picture of a patient’s life, tailor interventions to their home and resources, and watch their progress over time.
