Our Adult Medical Day Care is seeking a dedicated and compassionate Certified Social
Worker to join an amazing program and professional healthcare team. Your energetic
and professional approach will help foster a positive environment for participants, family
and colleagues alike.

Position is 8 AM to 4:00 PM with an hourly wage of $25 + an hour
And a sign on bonus of $500.
NO WEEKENDS, EVENINGS, OR NIGHTS
Applicants with either Adult Medical Day Care or long-term care experience is a plus.

Responsibilities not limited to the following
* Interviewing the participant and caregiver to obtain a social assessment and
evaluation of needs and problems.
* Assessing the government authority with completion of documents required by the
new jersey adult daycare, such as an application etc.
* Quarterly verification of participants Medicaid eligibility.
* Medicaid, food stamps, application and renewal housing application etc.
* Care plan development quarterly.
* Assisting participants with their medical and other billing problems.
