Job Description
Primary function.
Works in the Family Medicine clinic providing direct patient care to support the patient and healthcare practitioner. The Clinic Lead LPN is responsible for rooming patients, taking vitals, recording medical histories, reconciling home medications, administering medications as directed by the provider, and assisting with medical procedures. They are responsible for performing basic lab tests, preparing specimens, and drawing blood for testing. Clinic Lead LPN will be responsible for any other office duties assigned by the provider or clinic director. Additional duties may include but are not limited to prior authorizations, scheduling appointments, answering and returning phone calls, refilling medication requests, restocking rooms, and ordering clinic supplies.
Duties and responsibilities
Direct Patient Care Responsibilities: Provide patient care under the supervision of the physician or Nurse Practitioner.
Perform patient rooming to include:
Collection and recording vital signs, height, weight, chief complaint
Collecting a brief relevant history including pain status, allergies, current medication
Addressing any other pertinent information for the specific age groups
Report rooming information to the provider for examination
Performs specific care based on individualized medical care plans.
Performs patient assessment in accordance with chief complaint and presentation.
Identifies abnormal symptoms/changes in patients’ clinical, mental, or emotional condition, and reports findings to the provider.
Assist in patient procedures with the provider as determined by policy and standard of care.
Complete all POC testing as directed by the clinic provider.
Collects lab specimen as directed by the clinic provider.
Assist with patient admission and discharge processes as directed by the provider.
Provides and explains discharge instructions as directed by the provider.
Administers medications and immunizations orally, intramuscularly, subcutaneously, intradermally, and by inhalation as ordered by the provider, asking for assistance from the LPN or provider if unfamiliar with medication or procedures.
Maintains and performs all patient documentation in a timely, and factual manner
Utilizes only approved abbreviations in all documentation.
Accurately reflects the care provided.
Follow all policies and procedures related to documentation requirements.
Performs Emergency treatment as required.
Follows emergency policies and procedures and standards of BLS.
Promotes and ensures patient safety in all aspects of responsibilities.
Maintains a clean exam room to include procedure tables, equipment, and other work areas following current infection control and/or isolation procedure guidelines.
Routinely stocks the nurse’s station and exam rooms with supplies. Maintains an inventory of specialty items and initiates reorder of supplies as needed.
Routinely examines supplies for expiration and properly replaces expired materials as necessary.
Reports non-functioning or defective equipment to the supervisor immediately.
Follows procedures and protocols to ensure safe administration of medications and monitoring for adverse effects.
Utilizes proper body mechanics, transfer/lifting techniques to minimize fall risk to patients and injury to self.
Implements infectious disease controls as appropriate including isolation procedures, proper use of hand hygiene and gloves, specimen collection and transport, and during cleaning processes of rooms or equipment/instruments.

Qualifications
Qualifications include:
- Current LPN license in the state of Missouri
- 1-2 years' service with CHF Family Medicine Clinics
- BLS is required.
- Willingness and ability to work in all 6 clinic locations as needed
- Dependable, responsible, courteous, and tactful.
- Dedication to maintaining confidentiality of all patient records
Working conditions
Incumbents will be exposed to viruses, diseases, and infections from patients.
Serve multiple Age groups – neonatal, pediatric, adolescent and geriatric.
Physical requirements
While performing the duties of this job, the employee is regularly required to speak and listen. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping, and crouching as well as lifting and supporting patients when necessary.
