Must provide proof of required vaccinations and health screenings including Fit Test, Trinity 10 Panel Drug Screen, COVID-19 vaccine or exemption, MMR, Hepatitis B series, Influenza vaccine (optional), 2 Step PPD or IGRA, Varicella, and Physical/Health Statement within last year.
Must complete Trinity Skills Checklist for clinical contractors.
