As a Per Diem LPN, you'll work closely with our interdisciplinary team to deliver high-quality, patient-centered care in homes, assisted living communities, and other care settings.

- Provide direct patient care under the direction of the RN Case Manager and physician
- Administer medications and treatments as ordered
- Monitor patient conditions and promptly communicate changes to the clinical team
- Assist with admissions, consents, and coordination of care activities
- Provide patient and family education regarding disease progression, symptom management, and comfort measures
- Support implementation of individualized hospice care plans
- Maintain accurate and timely documentation within the electronic medical record
- Collaborate with nurses, hospice aides, social workers, chaplains, and other interdisciplinary team members
- Active Nevada LPN license in good standing
- Minimum one year of nursing experience preferred
- Hospice, palliative care, home health, long-term care, or skilled nursing experience strongly preferred
- Excellent communication and patient care skills
- Ability to work independently while maintaining strong team collaboration
- Valid driver's license, reliable transportation, and proof of insurance
