As a Per Diem LPN, you'll work closely with our interdisciplinary team to deliver high-quality, patient-centered care in homes, assisted living communities, and other care settings.

Position Responsibilities

• Provide direct patient care under the direction of the RN Case Manager and physician
• Administer medications and treatments as ordered
• Monitor patient conditions and promptly communicate changes to the clinical team
• Assist with admissions, consents, and coordination of care activities
• Provide patient and family education regarding disease progression, symptom management, and comfort measures
• Support implementation of individualized hospice care plans
• Maintain accurate and timely documentation within the electronic medical record
• Collaborate with nurses, hospice aides, social workers, chaplains, and other interdisciplinary team members

Qualifications

• Active Nevada LPN license in good standing
• Minimum one year of nursing experience preferred
• Hospice, palliative care, home health, long-term care, or skilled nursing experience strongly preferred
• Excellent communication and patient care skills
• Ability to work independently while maintaining strong team collaboration
• Valid driver's license, reliable transportation, and proof of insurance
