We are seeking a compassionate, skilled, and motivated Clinical Case Manager to join our 44 NORTH team. This role offers a unique opportunity to combine clinical therapy, case management, and supportive housing services while working with individuals experiencing serious mental illness and co-occurring conditions.

As a Clinical Case Manager, you will play a vital role in helping residents maintain housing stability, improve mental health, strengthen life skills, and achieve their personal recovery goals. You'll work as part of a multidisciplinary team dedicated to providing comprehensive, person-centered care in a supportive housing environment.

Pay: $74,000-$86,750

44 NORTH provides supportive housing and clinical services to individuals living with serious mental illness and other co-occurring conditions. Our team utilizes Housing First, trauma-informed, recovery-oriented, and strengths-based approaches to help residents build stability, independence, and long-term success.

Residents receive individualized support through therapy, case management, life skills development, and connections to community resources

What You'll Do

As a Clinical Case Manager, you'll provide both therapeutic and case management services to support residents in achieving their individualized goals.

Clinical Services
  • Conduct mental health assessments and screenings
  • Develop individualized service plans with residents
  • Provide individual therapy sessions as clinically appropriate
  • Facilitate psychotherapy and life skills groups
  • Utilize evidence-based practices including Motivational Interviewing and trauma-informed care
  • Document clinical services in KIPU and other required systems
  • Apply DSM-5-TR criteria when assessing and diagnosing residents
Case Management Services
  • Assist residents in accessing healthcare, employment, education, housing resources, and community support
  • Coordinate services with community providers and partner agencies
  • Advocate for residents and assist with navigating complex systems
  • Support residents in achieving service plan goals and maintaining housing stability
  • Conduct outreach and engagement activities to encourage participation in services
  • Assist with resident eligibility screenings and admissions processes
  • Maintain communication with residents on waiting lists and support applicants as needed
Team Collaboration
  • Collaborate closely with clinical, medical, housing, and support staff
  • Participate in multidisciplinary team meetings
  • Assist the Program Manager with administrative and operational tasks
  • Support program compliance, documentation, and quality standards
  • Serve as a positive role model for residents through professional and recovery-oriented interactions

Qualifications

Required

  • Master's Degree in Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or a related behavioral health field
  • Active Utah licensure as:
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Clinical Mental Health Counselor (LCMHC)
    • Licensed Marriage and Family Therapist (LMFT)
  • Case management experience
  • Strong clinical assessment, documentation, and treatment planning skills
  • Excellent interpersonal, communication, and problem-solving abilities

Preferred

  • Experience working with individuals experiencing serious mental illness
  • Experience in supportive housing or community mental health settings
  • Experience with co-occurring mental health and substance use disorders
  • Substance use disorder treatment experience
  • Familiarity with KIPU Electronic Health Record
