Positional Overview
The Imagine Group is recruiting for a Patient Care Navigator on behalf of our client, a leading healthcare organization in Buffalo, New York, this provider delivers comprehensive medical services through a network of hospitals, specialty centers, and outpatient facilities. Dedicated to excellence in patient care, innovation, and community health, the organization offers employees the opportunity to make a meaningful impact while working in a collaborative and mission-driven environment.
In this role, you will be responsible for supporting patients and providers in Gastroenterology, Colorectal/General Surgery or Cardiology. This is a role in a centralized contact center, and the support is generally provided through inbound and outbound calling as well as digital communications.
Role & Responsibility:
Tasks That Will Lead to Your Success
  • Answer inbound calls from external providers, hospitals or patients who have referrals or new patient requests
  • Schedule patient consults and procedures
  • Document calls, clinical escalations and patient needs in the medical record
  • Process referrals from inbound faxes and work queues
  • Make outbound calls to patients for reminders, procedure coordination and engagement
  • Work collaboratively with Contact Center managers, Practice Managers, Supervisors and Team Leads to constantly improve service levels and workflows
  • Collaborate with the Centralized Referral Management team to coordinate patient care
  • Perform other duties as assigned by management
Skills & Experience
Qualifications That Will Help You Thrive
  • High School Diploma or Equivalent
  • 2-3 years in a call center customer service environment, preferably in a healthcare environment
  • Specialty experience strongly preferred: Gastroenterology, Colorectal/General Surgery or Cardiology
  • Previous healthcare experience required
  • Basic medical terminology knowledge
  • EMR experience required, preferably with Epic
