We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) to join our hospice team and make a meaningful difference in the lives of patients and their families.
As a Hospice CNA, you will provide hands-on personal care and emotional support to patients in their homes, assisted living communities, and skilled nursing facilities while working alongside an interdisciplinary team committed to exceptional end-of-life care.

Responsibilities
Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance
Assist patients with activities of daily living while promoting comfort, dignity, and independence
Observe and report changes in patient condition to the supervising nurse
Provide emotional support and companionship to patients and families
Assist with light housekeeping related to patient care needs
Maintain accurate documentation of services provided
Collaborate with nurses, social workers, chaplains, and other hospice team members to support the patient's plan of care
Qualifications
Current CNA certification required
Hospice, home health, caregiving, assisted living, or skilled nursing experience preferred
Compassionate, patient-centered approach to care
Strong communication and interpersonal skills
Ability to work independently while maintaining close communication with the clinical team
Valid driver's license, reliable transportation, and proof of insurance
