44 North Case Manager
We are seeking a compassionate, motivated, and recovery-oriented Case Manager to join our 44 North team. This role provides an opportunity to support individuals experiencing serious mental illness and co-occurring conditions as they work toward housing stability, recovery, and community integration.
As a 44 North Case Manager, you will play a vital role in helping residents maintain stable housing, access community resources, develop life skills, and achieve their self-identified goals. You'll work alongside a multidisciplinary team dedicated to providing person-centered, trauma-informed, and recovery-focused services.
Salary: $46,353.60 annually

44 North provides supportive housing and recovery-focused services to individuals living with serious mental illness and co-occurring conditions. Our team utilizes Housing First, trauma-informed, strengths-based, and recovery-oriented approaches to help residents build stability, independence, and long-term success.
Residents receive individualized support through case management, life skills development, community integration activities, and connections to healthcare, employment, education, and other essential resources.
What You'll DoAs a 44 North Case Manager, you'll provide comprehensive case management and supportive services designed to help residents thrive in the community.
Case Management ServicesCollaborate with residents to identify needs and develop individualized service plans
Meet regularly with residents to review goals, progress, and evolving needs
Support residents in maintaining housing stability and complying with housing requirements
Assist residents with housing recertifications and required documentation
Connect residents to healthcare, employment, education, benefits, and community resources
Assist residents in obtaining and maintaining eligibility for Medicaid, SNAP, and other benefits
Provide client advocacy and support navigation of complex systems
Utilize assertive outreach and engagement strategies to encourage participation in services
Develop and facilitate life skills groups
Coordinate transportation for appointments, community activities, and special events
Support resident participation in activities that promote independence and community engagement
Utilize motivational interviewing techniques to support self-directed recovery goals
Apply Housing First, trauma-informed, and strengths-based approaches in all interactions
Maintain accurate and timely documentation in KIPU, HMIS, Azure, and other required systems
Complete case notes, assessments, care plans, and related documentation within required timeframes
Ensure compliance with HIPAA, 42 CFR Part 2, and organizational policies
Complete HRSS documentation, billing requirements, and fund requests as applicable
Maintain organizational productivity and documentation standards
Collaborate effectively with clinical, housing, medical, and community partners
Participate in team meetings, supervision, and required trainings
Assist the Program Manager with administrative and program support tasks as needed
Serve as a positive role model through professional, recovery-oriented interactions
Build meaningful relationships with residents and support their recovery journey
Help individuals maintain stable housing and improve quality of life
Work within a supportive multidisciplinary team
Utilize evidence-based engagement strategies including Motivational Interviewing
Make a direct impact on reducing homelessness and improving community integration
Join a mission-driven organization dedicated to changing lives
Bachelor's Degree in a human services-related field
Active Utah Social Service Worker (SSW) license
Case management experience
Strong interpersonal, communication, and problem-solving skills
Ability to work effectively with individuals experiencing serious mental illness and co-occurring disorders
Strong organizational skills and attention to detail
Ability to document services accurately and professionally
Proficiency with Microsoft Office and electronic documentation systems
Reliable attendance and punctuality
Experience working with individuals experiencing serious mental illness
Experience with substance use disorders
Experience working with individuals experiencing homelessness
Familiarity with Housing First and recovery-oriented models of care
Experience facilitating groups and community-based services
Familiarity with KIPU and HMIS systems
