Job Description
The Certified Nursing Assistant (CNA) provides compassionate, hands-on personal care and supportive services to hospice patients under the director of a Registered Nurse (RN). The CNA promotes comfort, dignity, and quality of life while supporting patients and their families during the end of life journey in accordance with the hospice plan of care and Acara Hospice's mission of providing "A Care Above."
Education
- High school diploma or equivalent preferred.
Licensure/Certification
- Current Texas Certified Nurse Aide (CNA) certification in good standing.
- Current CPR certification
- Valid Texas driver's license with an acceptable driving record.
- Current automobile insurance and reliable transportation
Experience:
- Hospice, home health, long-term care, and patient care experience preferred.
Duties & Responsibilities:
- Provide personal care including bathing, grooming, dressing, oral hygiene and toileting assistance
- Assist patients with ambulation, transfers, and positioning.
- Assist with feeding and encourage adequate nutrition and hydration as directed.
- Maintain a clean, safe, and comfortable environment for patients.
- Observe and report changes in the patient's condition, skin, pain or emotional status to the supervising RN.
- Provide companionship, emotional support, and compassionate presence to patients/families.
- Complete all required documentation accurately and timely.
- Maintain patient confidentiality in accordance with HIPAA regulations.
- Communicate effectively with the interdisciplinary team regarding patient needs and concerns.
Physical Requirements:
- Ability to lift, transfer, and reposition patients using proper body mechanics.
- Ability to stand, walk, bend, stoop, kneel, and reach.
- Ability to lift up to 50 pounds with or without accomodation.
Team Collaboration:
- Participate as an active member of the interdisciplinary hospice team.
- Attend required meetings, in-services, and educational programs.
- Follow all infection control, safety, and emergency procedures.
- Demonstrate professionalism and respect in interactions with patients, families, referral sources, and co-workers.
