CNY Family Care's commitment to excellence sets us apart and guides us as we provide care for our community.  The Dieteic and Diabetes Prgram Care Assistant (DDPC) assists the Dietetic-Diabetes Care Team at CNY Family Care by performing patient support tasks and serving as a primary administrative resource for the Registered Dietitian (RD) and/or CDCES (Certified Diabetes Care and Education Specialist). Key responsibilities include coordinating patient scheduling, triaging patient and provider inquiries, processing diabetes-related supplies and durable medical equipment orders and supporting access to diabetes care services. Works cooperatively and communicates effectively within a multidisciplinary framework to actively promote holistic care for patients and families.

Dietetic and Diabetes Program Care Assistant responsibilities

  • Appointment management of the Dietetic-Diabetes Care Team. 
  • Directs patients for procedures/testing as ordered by the physician and/or the RD/CDCES. 
  • Gives patient and family information/education materials as directed by the RD/CDCES/Physician for the success of overall patient care. 
  • Assists patients in navigating access to care services such as insurance coverage, prior authorization for diabetes prescription and services, Patient Assistance programs, community resources. 
  • Coordinates compliance of Diabetic Retinopathy quality measures for entire value-based payor population and provides reporting on data relevant to the assigned patient population. 
  • Manages the diabetic retinal eye exam schedule daily and administer patient exams.
  • Runs reports to identify patients due for an annual retinal eye exam and adds alert to appointment comment line. 
  • Reviews results for diabetic retinal eye exams performed daily and adds appropriate billing codes to E-superbill. 

Dietetic and Diabetes Program Care Assistant education/experience

  • High school diploma or general education degree (GED) required; one to two years of college preferred.
  • Minimum of two years previous experience working in Health Care required; or three or more years of related experience and/or training in a medical office which uses an Electronic Medical Record; or equivalent combination of education and experience.
  • Prior experience using Retinavue technology preferred.
