Travel Home Health Registered Nurse (RN) – St. Albans, VT
\n 📍 Facility: VNA Hospice of the Southwest Region – St. Albans, VT
\n 📅 Contract: 13 Weeks (Start ASAP)
\n 🕒 Schedule: Tuesday–Saturday | 8:00 AM – 4:00 PM
\n ⏰ Guaranteed Hours: 37 Hours/Week
\n VNA Hospice of the Southwest Region is seeking an experienced Home Health Registered Nurse (RN) for a 13-week travel assignment. This position provides skilled home health nursing services to patients in their homes while serving as a primary case manager. The ideal candidate is experienced in OASIS documentation, case management, and home health nursing, with the flexibility to provide hospice coverage during on-call rotations.
\n Position Highlights:
\n 13-week assignment
\n Tuesday–Saturday, 8:00 AM – 4:00 PM
\n 37 guaranteed hours per week
\n On-call rotation required (includes hospice coverage)
\n Mileage reimbursement available per agency policy
\n McKesson EMR
\n Holiday coverage required
\n Overtime after 40 hours per week
\n Responsibilities:
\n Provide comprehensive skilled nursing care in patients' homes using the nursing process
\n Complete accurate and timely OASIS assessments and documentation
\n Develop, implement, and evaluate individualized plans of care
\n Serve as the primary case manager, coordinating care with physicians, therapists, social workers, and other healthcare professionals
\n Perform patient assessments, medication management, IV therapy, wound care, wound VAC management, and other skilled nursing procedures
\n Educate patients and caregivers on medications, disease management, treatments, and discharge planning
\n Supervise and collaborate with LPNs and Home Care Aides as appropriate
\n Coordinate safe patient discharges and transitions of care
\n Participate in case conferences, peer reviews, and quality improvement initiatives
\n Maintain accurate documentation in McKesson EMR
\n Provide hospice patient coverage during on-call rotations as needed
\n Requirements:
\n Active Vermont RN License (or eligible to obtain upon acceptance)
\n Minimum 2 years of Home Health RN experience
\n Strong OASIS documentation experience required
\n Home Health case management experience preferred
\n Experience with IV therapy, wound care, wound VACs, and high-tech home care
\n Current BLS certification
\n Excellent assessment, communication, and organizational skills
\n Reliable personal vehicle and valid driver's license required for travel between patient homes
