Travel Pharmacist – Hospital Based Outpatient Infusion Center

Responsibilities

\n

\n
• Verify outpatient infusion and oncology medication orders.
\n
• Review treatment plans and IV infusion compounding.
\n
• Monitor medication therapy and provide patient education.
\n
• Manage infusion-related adverse reactions.
\n
• Collaborate with the interdisciplinary care team.
\n

\n

Requirements

\n

\n
• Active Oregon Pharmacist License.
\n
• Minimum 1 year of outpatient infusion pharmacy experience.
\n
• Oncology pharmacy/patient care experience required.
\n
• Experience with IV compounding and Epic EMR.
\n
