Home Health RN – Job Breakdown

Position: Registered Nurse (Home Health)
Shift: Days – 5x8s (7:00 AM – 3:00 PM)
Contract Length: 13 Weeks
Facility: Adventist Health Home Health & Hospice
Agency: Aya Healthcare
Account Manager: Emily Harris

Key RequirementsRequired Experience

• Minimum 2 years of Home Health RN experience
• First-time travelers accepted
• Home Health experience is mandatory
• Acute care experience preferred
• Experience managing an independent patient caseload

Required Licenses & Certifications

• Active RN License in state of practice
• BLS/CPR Certification
• Valid Driver's License
• Reliable vehicle
• Current automobile insurance

Strongly Preferred

• BSN
• Cerner EMR experience (required by facility for all travelers)
• Home care and case management experience

Daily ResponsibilitiesPatient Care

• Perform Start of Care (SOC), Recertifications, Resumptions of Care (ROC), and routine visits
• Conduct comprehensive patient assessments
• Develop and update care plans
• Monitor patient progress and identify changes in condition
• Provide skilled nursing interventions

Education & Coordination

• Educate patients and families
• Coordinate care with PT, OT, ST, Social Work, and providers
• Participate in interdisciplinary care planning

Documentation

• Complete accurate documentation in Cerner
• Update medication profiles
• Submit all documentation timely
• Maintain compliance with HIPAA and agency standards
