Licensed Practical Nurse (LPN)
Location: Grand Junction, CO
Type: Contract (13 Weeks, Possible Extension)
Pay: $40/hr – $49/hr
Shift & Schedule: Night Shift | 7:00 PM – 7:00 AM | 36 Hours/Week
Position OverviewWe are seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate, resident-centered care in a Long-Term Care and Skilled Nursing setting. The LPN will work closely with residents, families, and the interdisciplinary healthcare team to ensure quality patient care and support resident well-being.
Responsibilities• Provide direct nursing care to residents in a long-term care and skilled nursing environment.
• Administer medications, treatments, and nursing interventions as prescribed.
• Monitor residents' conditions and report changes to the healthcare team.
• Assist with admissions, assessments, care planning, and documentation.
• Support residents with rehabilitation and recovery goals.
• Perform wound care, basic treatments, and other nursing procedures within scope of practice.
• Maintain accurate and timely documentation in the electronic medical record system.
• Collaborate with physicians, RNs, CNAs, therapists, and other healthcare professionals.
• Promote resident safety, dignity, comfort, and quality of life.
• Educate residents and families regarding care plans and treatment goals.
• Follow facility policies, infection control guidelines, and regulatory requirements.
Required Qualifications• Active LPN License.
• Current BLS/BCLS Certification.
• Minimum 1 year of LPN experience required.
• Previous experience in Long-Term Care (LTC), Skilled Nursing (SNF), Rehabilitation, or Geriatric Care preferred.
• Experience with EMR systems preferred.
• Strong communication, documentation, and patient care skills.
Preferred Qualifications• Long-Term Care or Skilled Nursing Facility experience.
• Medication administration experience.
• Patient assessment and monitoring skills.
• Strong teamwork and organizational abilities.
