- Monitor patient progress and adjust treatment plans to optimize outcomes
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
- Maintain accurate and timely patient documentation in compliance with regulatory requirements
- Support patients in achieving greater mobility, function, and overall well-being within their home environment
- Minimum of nine months of experience as a Physical Therapist
- Home health or community-based therapy experience preferred
- Master's degree in Physical Therapy from a CAPTE-accredited Physical Therapist education program
- Current Physical Therapist license in the state of practice
- Valid CPR certification maintained throughout employment
- Valid driver's license and ability to travel to patient locations
- Strong clinical assessment, communication, and patient education skills
