SCHOOL-BASED HEALTH LICENSED PRACTICAL NURSE (LPN)


This LPN position is located in the Coconut Grove / Coral Gables area


POSITION PURPOSE

The School-Based Health Licensed Practical Nurse (LPN) is an essential member of CHI's School-Based Health Program and works collaboratively with students, families, school personnel, healthcare providers, behavioral health professionals, and community partners to support student health and wellness.

Under the supervision of a Registered Nurse, Advanced Practice Registered Nurse, Physician, or other licensed healthcare provider, the School-Based LPN provides direct nursing care, medication administration, chronic disease monitoring, preventive health services, telehealth support, care coordination, and health education. The LPN assists in ensuring students have timely access to healthcare services that promote health, well-being, school attendance, and academic success.


EDUCATION AND EXPERIENCE

Education

  • Graduate of an accredited Practical Nursing Program.

Experience

  • Minimum of one (1) year of nursing experience preferred.
  • Experience in pediatrics, family medicine, community health, public health, school health, ambulatory care, or primary care preferred.
  • Experience working with pediatric and adolescent populations preferred.
  • Experience in a Federally Qualified Health Center (FQHC), school-based health program, public health department, or community health setting preferred.

Licensure and Certification

  • Current Florida Licensed Practical Nurse (LPN) license in good standing.
  • Current Basic Life Support (BLS) certification through the American Heart Association.
  • CPR certification required.

Skills and Abilities

  • Knowledge of pediatric and adolescent health issues.
  • Strong communication and interpersonal skills.
  • Ability to work effectively with students, families, school personnel, and healthcare providers.
  • Ability to recognize changes in health status and report concerns appropriately.
  • Proficiency in electronic health record documentation.
  • Ability to work independently within established protocols and scope of practice.
  • Bilingual English/Spanish and/or English/Haitian Creole preferred.


POSITION RESPONSIBILITIES

Direct Student Care

  • Provide nursing services to students within the scope of LPN licensure and under appropriate clinical supervision.
  • Obtain and document vital signs, height, weight, BMI, vision screening results, and other clinical information.
  • Perform student health assessments and observations and report abnormal findings to the supervising RN or healthcare provider.
  • Assist in the management of students presenting with illness, injury, or health concerns.
  • Provide first aid and nursing interventions consistent with CHI policies and procedures.
  • Respond appropriately to urgent and emergency situations and notify supervising clinical staff as required.

Medication Administration and Treatment Services

  • Administer medications and treatments in accordance with physician orders, standing orders, organizational policies, and applicable regulations.
  • Monitor students receiving medications and report adverse reactions or concerns to the supervising clinician.
  • Maintain accurate medication administration records.
  • Assist in monitoring students with chronic health conditions requiring ongoing treatment or observation.

Chronic Disease Management and Care Coordination

  • Assist with implementation and monitoring of individualized care plans for students with chronic health conditions.
  • Support management of students with asthma, diabetes, seizure disorders, sickle cell disease, behavioral health conditions, and other chronic illnesses.
  • Reinforce treatment plans and health education provided by licensed providers.
  • Assist students and families in accessing healthcare services and community resources.

Preventive Health Services

  • Conduct preventive health screenings, including vision, blood pressure, height, weight, and BMI screenings.
  • Assist with oral health initiatives, fluoride varnish programs, immunization compliance activities, and health promotion efforts.
  • Support identification of students requiring referral for additional medical, dental, behavioral health, or vision services.
  • Promote healthy behaviors and wellness activities among students.

Telehealth Support

  • Facilitate telehealth visits between students and CHI medical and behavioral health providers.
  • Prepare students for telehealth encounters and assist providers during virtual visits.
  • Ensure telehealth equipment is operational and functioning appropriately.
  • Maintain student privacy and confidentiality during telehealth services.

School-Based Operations and Collaboration

  • Serve as a liaison between CHI, students, families, and school personnel.
  • Collaborate with school administrators, school nurses, counselors, teachers, and other staff to support student health needs.
  • Participate in school meetings and multidisciplinary team discussions as assigned.
  • Promote awareness and utilization of school-based health services.
  • Support student enrollment and consent form collection activities as needed.

Documentation, Quality, and Compliance

  • Maintain accurate, timely, and complete documentation within the electronic health record (EPIC).
  • Ensure compliance with HIPAA, FERPA, grant requirements, organizational policies, and applicable regulations.
  • Participate in documentation audits, quality improvement activities, and performance improvement initiatives.
  • Assist with collection and reporting of required program data and performance metrics.

Program Operations

  • Maintain clinical supplies, equipment, and inventory at assigned school sites.
  • Ensure examination and treatment areas remain organized, clean, and appropriately stocked.
  • Follow infection prevention and control procedures, safety protocols, and emergency preparedness requirements.
  • Participate in staff meetings, training activities, competency assessments, and professional development opportunities.
  • Assist with implementation of operational workflows and program initiatives.
