Contingent Home Health Medical Social Worker (MSW) Compensation:

• Visit Rates with Mileage Reimbursement: $65.00 - $75.00 per visit
• Discharge Without a Visit: $5.00 - $10.00 per non-visit discharge
• Transfer Without a Visit: $5.00 per non-visit transfer
• Orientation and In Service Hours: $30.00 per hour
• Non taxed mileage reimbursement is paid at $0.50 per mile after 30 miles per day
• Visit Rates without Mileage Reimbursement: $70.00 - $80.00 per visit

Home Health Medical Social Worker (MSW) Job Description:

• Provides social work services in accordance with the physicians plan of treatment.
• Assists the physician and other home health team members in understanding the significance of social and emotional factors related to the patients care.
• Assesses and reassesses social and emotional factors to help the patient and family cope with daily living problems and to help them follow medical recommendations to maximize the patients health condition.
• Counsel patients and families concerning the facilitation of the medical plan of treatment.
• Recognizes and utilizes community and family resources to assist the patient in long-term plans for life in the community or to learn to live within the patients disability.
• Develops the care plan with the case manager, physician, patient, and family to deal with personal, financial, and environmental difficulties that may interfere with the health and wellness continuum. Sets appropriate goals for the patient. Participates in discharge planning.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and according to policies.

Home Health Medical Social Worker (MSW) Requirements:

• Active Michigan Medical Social Worker (MSW) license
• Reliable transportation
• Valid driver's license and auto insurance in your name as a driver
• Home health care experience preferred

Home Health Medical Social Worker (MSW) Degree of Travel:

• Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
