Position: Jail Mental Health Program - Case Manager (Grand Rapids, MI)

Location: Kent County Correctional Facility

Position Type: Full-Time

Salary: $56,000

We are seeking a hard working and motivated candidate to work with incarcerated individuals at the Kent County Correctional Facility. The ideal candidate will have a bachelor's degree in Social Work or a related field with two or more years of relevant experience serving diverse populations. Knowledge of the criminal justice system is desired. Applicants will need to pass a background check prior to hire.

Responsibilities include:

• Collect, compile, and maintain accurate emergency and resource information for utilization in work with adult inmates, juveniles, and their families.
• Serve as a referral advocate for those needing technical assistance in accessing community resources key to healthy functioning.
• Maintain and provide information/training in self-advocacy, rights, and services for inmates/families.
• Complete assessment forms, progress notes, weekly and monthly reports as required.
• Meet with inmates regularly and update outside case managers on court dates, bonds, mental health, and other concerns.
• Coordinate with inmates on release plans including transportation, housing, and continuing mental health treatment.
• Collect, and compile information for monthly reports
• Work collaboratively with other members of the Jail Mental Health Team and Kent County Jail staff.
