Patient Management Nurse

The Patient Management Nurse plays a vital role in coordinating care for patients using the eClinicalWorks electronic health record system. This position involves assisting patients with acute medical needs, identifying high-risk individuals, and providing transitional care by connecting patients with community resources. The nurse also addresses healthcare barriers and participates in patient education events to improve health outcomes.

Responsibilities

• Coordinate patient care and follow-up in hospital and emergency settings
• Provide patient education and participate in education events
• Collaborate effectively with medical teams and providers
• Address patient inquiries and triage urgent needs
• Manage referrals and appointments scheduling
• Assist providers with clinical tasks and maintain provider scorecards
• Perform phlebotomy, vital signs monitoring, and medication administration
• Obtain and implement physician orders
• Identify and resolve healthcare barriers impacting patient care
• Connect patients with community resources for ongoing support
• Maintain multitasking and organizational skills in a fast-paced environment

Preferred Qualifications

• Minimum of 1 year nursing experience
• Licensed Practical Nurse (LPN) License or Registered Nurse (RN) License
• Proficiency in patient care and medication administration
• Experience with Electronic Health Records (EHR), specifically eClinicalWorks
• Skilled in vital signs monitoring and phlebotomy
• Strong communication skills
• Experience with insertion and removal of Foley catheters
• Competence in ear lavage and IV fluid administration
