Licensed Vocational Nurse (LVN) – Six-Bed Congregate Living Health Facility

We are seeking a compassionate and dependable LVN to join our care team at our six-bed Congregate Living Health Facility in Victorville, CA. This position offers the opportunity to provide high-quality, individualized care in a supportive and structured home-like environment.

Responsibilities:

• Provide daily nursing care and monitoring for residents with chronic or complex medical needs.

• Administer prescribed medications and treatments accurately and in accordance with physician orders.

• Monitor vital signs, report changes in condition, and document all care in compliance with Title 22 and facility policies.

• Collaborate with the interdisciplinary team (RN, physicians, administrators, direct support staff) to ensure continuity of care.

• Maintain proper infection control practices, including sterilization of medical tools and maintaining a safe clinical environment.

• Support residents with health education and ensure families are kept informed of changes in care plans.

• Uphold dignity, respect, and compassion in all resident interactions.

Qualifications:

• Current California LVN license in good standing (required).

• Previous experience in skilled nursing, congregate living, long-term care, or a similar setting preferred.

• Strong clinical assessment skills and familiarity with medical tools and equipment.

• Ability to build positive rapport with residents, families, and staff.

• Excellent written and verbal communication skills.

• Compassionate, patient, and professional demeanor.
