About the Role:
Join HOME LINK INTERNATIONAL INC as an LPN in Home Care, where you'll provide compassionate, patient-centered care in the comfort of our clients' homes. We are excited to welcome dedicated nursing professionals who are passionate about making a difference in the lives of others.
Responsibilities:
- Conduct comprehensive patient assessments to develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients’ health status and report changes to the healthcare team.
- Educate patients and their families about health management and care procedures.
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
- Document patient care activities and maintain accurate records.
- Provide emotional support and companionship to patients and families.
- Adhere to safety and infection control protocols at all times.
Requirements:
- Current LPN license in the state of New Jersey.
- Minimum of 1 year of nursing experience, preferably in home care or community health.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor and a genuine desire to help others.
- Valid driver's license and reliable transportation.
- Basic Life Support (BLS) certification required.
