Licensed Practical Nurse (LPN) / Registered Nurse (RN) – Part-Time or PRN
South Prairie Memory Care | Bethalto, IL
South Prairie Memory Care is seeking compassionate and dependable Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team on a part-time or PRN basis.
Our nurses play a vital role in providing exceptional care to residents living with Alzheimer's disease and other forms of dementia. This position offers the opportunity to work closely with residents, families, caregivers, and leadership in a supportive, team-focused environment.
Position Summary
The LPN/RN practices in accordance with community policies and procedures under the direction of the Director of Nursing. Nurses are responsible for medication administration, resident assessments, clinical documentation, supervision of caregiving staff, and ensuring residents receive safe, compassionate, and high-quality care.
Responsibilities
- Administer medications and treatments in accordance with physician orders and community policies.
- Accurately document medication administration and resident care in the Electronic Medical Record (EMR).
- Monitor residents for changes in condition and communicate findings to the Director of Nursing and healthcare providers as appropriate.
- Complete resident assessments, progress notes, and required clinical documentation.
- Assist with admissions, resident evaluations, and service plan updates.
- Provide leadership, supervision, and support to Resident Assistants (RAs) and Certified Nursing Assistants (CNAs).
- Assist caregivers with resident care and Activities of Daily Living (ADLs) as needed.
- Communicate effectively with residents, families, physicians, pharmacies, and other healthcare providers.
- Respond promptly to resident emergencies and urgent clinical concerns.
- Participate in fall prevention, infection prevention, and quality assurance activities.
- Maintain accurate and complete medical records.
- Assist with medication ordering, medication reconciliation, and pharmacy communication.
- Monitor resident weights, vital signs, and overall well-being.
- Promote resident dignity, independence, comfort, and quality of life.
- Attend required in-services, staff meetings, and training programs.
Qualifications
- Current Illinois Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.
- Nursing experience preferred; experience in assisted living, memory care, geriatrics, or long-term care is a plus.
- Knowledge of dementia care and person-centered care practices preferred.
- Strong assessment, communication, and documentation skills.
- Ability to work independently and as part of a team.
- Ability to supervise and support caregiving staff.
- CPR certification preferred.
