Requirements

• Active, unrestricted Louisiana medical license
• DEA license with LA address
• LA Controlled Substance & Supervising Physician License (PA supervision)
• ABEM certification or EM board eligible with ACLS, ATLS, PALS (AHA preferred)
• Documentation of case logs for past 2 years
• CME/CEU proof for last 2 years
• Credentialed at Willis-Knighton Medical Center and affiliated facilities
