American Traveler is hiring a travel RN for a Home Health position in New Jersey, requiring 2+ years of home health experience, OASIS experience, and Homecare Homebase software proficiency.
Details- Home health setting serving patients in Union and Somerset County, NJ
- Caseload includes home health and hospice patients across all age groups
- Full-time day shift schedule: Monday–Friday, 8:00 AM–4:30 PM (7.5 hours/day, 38 hours/week)
- Epic EMR used; experience strongly preferred
- Homecare Homebase software experience required
- No on-call responsibilities
- Active NJ RN license (or compact license) required
- Current CPR/BLS certification required
- Current driver's license, reliable transportation, and auto insurance required
- Minimum 1–2 years of RN clinical experience required
- Minimum 2 years of home health experience required
- OASIS experience required
- Homecare Homebase software experience required
- Proficiency in wound staging and treatment, IV care, venipuncture, phlebotomy, inhalation therapy, catheter and drain maintenance, and bowel/bladder training programs
- Graduate of an accredited ADN or Nursing Diploma program required; BSN preferred
- CHPN certification preferred
- Two professional references required for consideration
- Candidates must reside more than 50 miles from the facility to be eligible
- Administer skilled nursing care to home health and hospice patients in their place of residence
- Develop, implement, document, and evaluate individualized Plans of Care (POC)
- Supervise and provide clinical direction to HHAs, STNAs, and LVNs
- Coordinate care with interdisciplinary teams, patients and families, physicians, and referring agencies
- Educate patients and families on care, medications, diet, and home safety using age- and culture-appropriate teaching methods
- Attend interdisciplinary team meetings as needed
- Floating to a nearby sister facility may be required
- Travelers who have been on assignment for one continuous year must take a 30–60 day break before beginning a new assignment at this facility
