Travel RN Care Manager | York, ME
13 Weeks | Days | 40 Hours/Week
We areseeking an experienced RN Care Manager for a 13-week assignment at its main campus.

Requirements:

• Active ME RN License
• Associate or Bachelor's Degree in Nursing
• 3-5 years of clinical experience in Med/Surg, Home Health, Case Management, or Utilization Review
• Must have Utilization Review and Discharge Planning experience
• Active BLS required
• CCM or ACM certification preferred

Schedule:

• 40 hours/week
• Variable shifts:

* 8:30 AM – 4:30 PM, or
* Four 10-hour shifts (7:30 AM – 5:00 PM)

• No call
• Weekend and holiday rotation required

Responsibilities:

• Coordinate utilization management and discharge planning
• Assess patient needs and secure post-acute resources
• Lead interdisciplinary care rounds and care planning
• Coordinate DME, transportation, and insurance authorizations
• Monitor length of stay and reimbursement requirements
