We are seeking an experienced Travel Physical Therapist (PT) for a 13-week Home Health assignment. The PT will provide skilled therapy services to patients in their homes and residential settings, helping them improve mobility, reduce pain, restore function, and maximize independence.
This position is ideal for a therapist who enjoys working autonomously while delivering high-quality, patient-centered care in the home health setting.
Assignment Details- Length: 13 Weeks
- Schedule: Days, Monday–Friday
- Missed visits may be made up on weekends as needed
- Guaranteed Hours: 40 hours per week
- Travel Radius: Up to 50 miles from the facility
- Expected Weekly Mileage: Approximately 50–75 miles (may vary)
- Mileage: Included in bill rate
- Patient Load: Approximately 6–7 visits per day
- Charting System: WellSky
- Required Daily Stand-Up Meeting: Monday–Friday at 8:30 AM
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide therapeutic exercises, gait training, balance training, manual therapy, and functional mobility interventions
- Monitor patient progress and modify treatment plans as needed
- Educate patients and caregivers on home exercise programs, safety strategies, and recovery plans
- Complete accurate and timely documentation in WellSky
- Coordinate care with physicians, nurses, therapists, and other healthcare professionals
- Participate in interdisciplinary care planning to ensure optimal patient outcomes
- Deliver compassionate, patient-centered care in home and residential settings
- Required: Graduate of an accredited Physical Therapy program
- Required: Active PT license in the state of assignment or ability to obtain
- Required: Current BLS and First Aid certifications
- Required: Minimum 2 years of recent licensed PT experience
- Required: At least 1 recently completed travel assignment
- Required: Experience with HomeCare/HomeBase
- Required: Home Health experience
- No New Graduates
- COVID-19 vaccination and booster(s) required
- Medical and religious exemptions considered on a case-by-case basis
- Approved exemptions require weekly testing per facility policy
