📋 Key Responsibilities: Provide direct nursing care, triage, and chronic condition monitoring for elderly/frail participants. Deliver specialized treatments including wound care, routine lab draws, and comprehensive nurse assessments. Conduct regular home visits and collaborate closely with Primary Care Providers (PCPs) to execute care plans. Document efficiently using the TruChart EMR system.
🔑 Requirements:
Experience: Minimum 3 year of experience working with geriatric/frail populations (Home health and wound care experience highly preferred).
Licensure: Active Michigan (MI) Registered Nurse license.
Certification: Active AHA BLS/BCLS certification.
Education: Associates Degree in Nursing required (BSN preferred).
Travel: Must have reliable transportation, a valid MI driver’s license, and active car insurance for home visits.
Health: Fully vaccinated (or willing to be) against Influenza, MMR, Varicella, Tdap, Covid-19, and Hep B.
💵 Compensation Details:
Guaranteed Hours: 40 hours/week
Overtime: Starts after 40 ours/week
