Certified Nursing Assistant (CNA)

Elderhaus PACE is seeking a compassionate, dependable, and participant-centered CNA to provide hands-on care and daily assistance to older adults in our PACE program.

Position Summary
Under the supervision of the Clinical Supervisor, RN, the CNA, provides direct participant care, and support with activities of daily living. This position helps participants reach and maintain their highest level of functioning while following physician orders, care plans, organizational policies, industry standards, and applicable federal and state regulations.
The CNA assists with restorative and supportive care, activity program implementation, participant monitoring, documentation, and maintaining a safe and respectful environment. This role also functions as a member of the interdisciplinary team and supports the overall care and well-being of Elderhaus PACE participants.
Key Responsibilities
The CNA will:
- Provide care according to Elderhaus PACE approved care plans, nursing orders, policies, procedures, and standards.
- Assist participants with activities of daily living, including bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care, toileting, and personal hygiene.
- Assist participants with walking, wheelchair use, self-exercise, transfers, and safe movement.
- Assist with lifting and transporting participants using proper lifting techniques.
- Monitor and assist participants with meals and nourishments and record consumption as needed.
- Provide bed pan, urinal, and incontinent care.
- Record participant weight, measurements, vital signs, food intake, output, and other assigned observations.
- Document care, participant activities, appearance, and relevant health information according to Elderhaus PACE policy.
- Notify the supervisor of participant changes, concerns, difficulties, errors, or occurrences.
- Note changes in a participant’s physical or emotional health and communicate this information to the appropriate staff person.
- Provide and maintain a safe environment for participants.
- Support quality improvement initiatives.
- Follow risk management, safety, infection control, employee TB screening, security, fire safety, OSHA, and other applicable requirements.
- Handle potentially infectious specimens with appropriate biohazard precautions and practice Universal Precautions.
- Maintain confidentiality of participant, client, family, and organizational information.
- Participate in continuing education classes, required staff meetings, and training.
- Perform other duties as assigned.
Qualifications
Qualified candidates should have:
- High school diploma or equivalent.
- Current Certified Nursing Assistant certification.
- At least one year of experience working with frail or elderly populations required.
- Valid driver’s license, proof of insurance, and reliable transportation.
- Medical clearance for communicable diseases and required immunizations prior to direct participant contact.
- Ability to complete required position-specific competencies before assuming participant care responsibilities.

Work Environment and Physical Requirements
This position works in a participant care environment serving older adults with complex needs. The role may involve direct personal care, participant transfers, mobility assistance, exposure to infection, and caring for individuals with various cognitive abilities.
The position requires the ability to communicate verbally with staff, participants, caregivers, families, and community partners. The role also requires standing, walking, occasional pushing, pulling, and lifting.
