Primary Registered Nurse (Hospice RN) to join our interdisciplinary hospice care team. The Primary RN is responsible for managing an assigned caseload of hospice patients, coordinating care plans, and providing skilled nursing services in various settings, including private residences, nursing homes, assisted living facilities, and group homes. The ideal candidate possesses strong clinical assessment skills, excellent critical thinking abilities, and a passion for educating and supporting patients and caregivers throughout their hospice journey.

• Manage and coordinate care for an assigned caseload of hospice patients.
• Perform comprehensive patient assessments and develop individualized care plans.
• Monitor, evaluate, and manage patient symptoms to ensure comfort and quality of life.
• Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
• Collaborate with physicians, social workers, chaplains, aides, therapists, and other interdisciplinary team members.
• Provide compassionate emotional support to patients and families.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings and care planning discussions.
• Ensure compliance with hospice regulations, standards, and organizational policies.
