Requirements:

2 years of experience as a RN
\n \n - \n
Qualified applicants MUST have at least 1-2 years of experience as a Registered Nurse in a clinical care setting

Qualified applicants MUST have Home health experience (2+ years), Homecare Homebase software experience, OASIS experience, CHPN certification, current state license as a Registered Nurse, proof of current CPR certification, and a valid driver’s license with reliable transportation and insurance

Preferred: 1) Bachelors of Science in Nursing, 2) preferred CHPN certification (as applicable), and 3) Proof of current CPR certification

Valid RN license

Be willing to obtain State licensure

Responsibilities: As a healthcare professional in a Clinical facility (home health setting)...

Assess home health and/or hospice patients in stable and emergent situations, identify physical/psychosocial/environmental needs, and document findings for continuity of care (RN)
\nImplement, develop, document, and evaluate the Plan of Care (POC), incorporating OASIS-informed needs and coordinating interdisciplinary care (RN)
\nProvide education to patients and families on care, diet, medications, and home safety; deliver infection control practices (OSHA) and maintain accurate patient records (RN)
