We are seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality, patient-centered care in the home health setting. This position is responsible for delivering individualized, goal-oriented nursing care utilizing the nursing process in accordance with physician orders. The RN will assess, plan, implement, and evaluate patient care while promoting independence, safety, and optimal health outcomes.

In addition to providing skilled nursing services, the RN serves as the Case Manager for assigned patients when nursing is the lead discipline, coordinating care across the healthcare continuum and collaborating with patients, families, physicians, and interdisciplinary team members.

 

Key Responsibilities

• Provide skilled nursing care in patients' homes in accordance with physician orders and established care plans.
• Conduct comprehensive patient assessments and develop individualized plans of care.
• Monitor patient conditions and evaluate responses to treatment and interventions.
• Administer medications, treatments, and other nursing services as prescribed.
• Educate patients and caregivers regarding disease management, medications, treatment plans, and self-care techniques.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Serve as Case Manager for assigned patients when nursing is the primary discipline.
• Develop, implement, and revise patient care plans to ensure optimal outcomes.
• Maintain accurate and timely clinical documentation.
• Ensure compliance with agency policies, state regulations, and professional nursing standards.
• Participate in on-call and weekend rotation schedules as assigned.

Required Qualifications

• Active Registered Nurse (RN) License
• Graduate of an accredited nursing program
• Strong clinical assessment and critical-thinking skills
• Excellent communication and patient education abilities
• Valid Driver's License and reliable transportation
• Ability to work independently in a home health environment

Preferred Qualifications

• Previous Home Health, Hospice, Case Management, Medical-Surgical, or Community Health Nursing experience
• Experience coordinating multidisciplinary care plans
• Familiarity with electronic medical record (EMR) systems
• OASIS documentation experience preferred
