Hultquist Place Assisted Living
Status: Full-Time, Non-Exempt
Schedule: 40 hours per week | 3rd Shift (10:00 PM – 6:30 AM) | Includes every other weekend
Pay: $25 - $30 per hour, commensurate with experience
Hultquist Place Assisted Living is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our dedicated healthcare team. In this role, you will provide high-quality nursing care to residents while supporting a safe, respectful, and person-centered environment.
As an LPN Charge Nurse, you will work under the supervision of the Director of Patient Services and RN Case Manager to ensure residents receive excellent clinical care consistent with facility policies, procedures, and New York State regulations. This role also includes leading and supporting the care team on shift to ensure quality resident care and effective team communication.
Shift Differential:
While working 3rd shift, Lutheran employees receive an additional $1.50/hour shift differential.
Retention Bonus:Eligible candidates may receive a $5,000 retention bonus, paid in installments over the first 24 months of employment.
Position Summary:The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to residents, assisting with medication administration, supervising care staff, and ensuring accurate documentation of all resident care activities. The LPN also plays an important role in recognizing changes in resident conditions and communicating with the interdisciplinary care team.
- Graduate of an accredited Practical Nursing program
- Current New York State LPN license in good standing
- Demonstrated interest in geriatric nursing and resident-centered care
- Ability to work effectively within an interdisciplinary healthcare team
- Strong communication, organization, and documentation skills
- Provide nursing care to residents in accordance with facility policies, procedures, and regulatory standards
- Monitor residents for changes in condition and report findings to the RN Case Manager
- Follow and implement individualized care plans
- Assist with direct resident care when needed
- Support a safe, comfortable, and respectful physical, social, and emotional environment for residents
- Assist with medication administration in accordance with facility policies
- Observe the six rights of medication administration
- Receive medications from the pharmacy and maintain accurate medication records
- Participate in medication review meetings and audits
- Maintain medication carts, medication rooms, and related documentation
- Accurately complete and maintain resident medical records and documentation
- Provide and receive shift reports
- Report incidents, accidents, and resident concerns promptly
- Communicate with physicians and providers regarding resident care and orders
- Ensure all documentation complies with regulatory and facility requirements
- Supervise and guide the day-to-day work of HHAs, PCAs, and other nursing staff
- Assist with staff coaching, evaluations, and performance feedback when assigned
- Conduct shift meetings to ensure effective communication among team members
- Participate in Quality Improvement, Safety, and Team meetings
- Complete incident reports and notify appropriate parties including providers and families
- Prepare shift assignments for care staff
- Maintain accurate records for appointments, orders, and daily communications
- Assist with audits and regulatory compliance activities
