Job Title: Home Health Physical Therapist – PT

Location: Wytheville, VA (In-Home Patient Care)

Schedule: PRN (Flexible Scheduling Available)

Pay: Competitive Per Visit Rate (varies on visit type) 

Job Overview:

We are currently seeking a compassionate and skilled Home Health Physical Therapist – PT to provide personalized, one-on-one therapy in a personal setting in Wytheville, VA. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and overall functionality in their daily environment.

As a Home Health Physical Therapist – PT, you will work closely with patients recovering from illness or injury, developing and adjusting care plans to support their goals and improve outcomes. This role requires travel to patient homes throughout the Wytheville and surrounding areas.

Responsibilities of Home Health Physical Therapist – PT:

  • Provide in house physical therapy to patients recovering from illness or injury
  • Conduct initial assessments and develop individualized treatment plans
  • Monitor patient progress and modify care plans as needed
  • Assist patients with exercises and hands-on therapy to improve mobility and manage pain
  • Educate patients on their plan of care, therapy goals, and self-care techniques
  • Coach family members on how to safely assist with care and exercises
  • Collaborate with interdisciplinary team members (RN, LPN, CNA, OT, SLP)
  • Maintain accurate and timely documentation

Qualifications of Home Health Physical Therapist – PT:

  • Graduate of an accredited Physical Therapist program
  • Active PT license in the state of Virginia
  • Minimum of 2 years of physical therapy experience (home health preferred)
  • CPR certification and TB baseline screening
  • Strong clinical judgment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently in a personal setting
  • Willingness to travel throughout the service area
